Waist-to-Height Ratio vs. BMI: Which One Should You Actually Track?

Waist-to-height ratio (waist circumference divided by height) predicts obesity-related health risk more accurately than BMI in most studies, because it captures where fat is stored rather than just how much of it there is. That doesn't make BMI useless — it makes the two numbers complementary, not competing.

What waist-to-height ratio actually measures

Divide waist circumference by height, using the same units for both. A target under 0.5 (waist less than half your height) is the widely cited healthy cutoff; some studies use slightly different thresholds by sex — around 0.53 for men and 0.54 for women for general obesity risk, and 0.59 for abdominal obesity risk in both.

The measurement's accuracy depends entirely on where the tape goes: around the narrowest part of the torso, typically just above the belly button, not wherever your pants happen to sit — vanity sizing means a '32 waist' pair of jeans is very often an actual 34 or 35 inches.

Why researchers increasingly prefer it to BMI

BMI treats a kilogram of visceral fat around the abdomen the same as a kilogram anywhere else on the body, but abdominal fat specifically is what's most strongly linked to diabetes, heart disease, and stroke risk. Waist-to-height ratio captures that distribution directly, which is why several large studies have found it a better predictor of these outcomes than BMI alone — one widely cited analysis found over a third more adults would be classified at elevated risk using waist-based measures than BMI alone flags.

This is also why two people with an identical BMI can have very different real health risk: someone carrying weight mostly around the abdomen is generally at higher risk than someone with the same BMI carrying it on the hips and thighs — a distinction BMI cannot see and waist-to-height ratio can.

Why BMI still isn't obsolete

BMI needs only a scale and a height measurement, making it far cheaper and faster to collect at population scale than a correctly measured waist circumference, which is why it remains the standard first-pass screening tool in most medical settings. It also has decades of research linking it to outcomes, giving it a depth of population-level data that newer metrics haven't fully caught up on yet.

The practical answer isn't choosing one over the other — it's using BMI as the fast first screen and waist-to-height ratio (or a full body-fat measurement, for anyone training seriously) as the follow-up that adds the distribution information BMI can't provide.
